Investigator Registration

Step 1: Please download and fill out the Researcher Portal Narrative Form. Email the completed form to patientregistry@foodallergy.org

Step 2: Please complete the fields below and click on the Register button.

The FARE team will review your application once complete and reserves the right to approve or deny your request for access.

In applying for access to the FARE Investigator Portal, you must agree to the following:

  • If approved, your username and password should be used ethically and responsibly, only for yourself and those in your organization with similar intent.
  • If you wish to publically share an analysis of the information included from the FARE Patient Registry, you will ensure that the information is being used in an ethical manner and notify FARE of the data you plan to share to help us track outcomes. Publically published findings must credit FARE as the source of data by listing the FARE Patient Registry, developed by Food Allergy Research & Education (FARE).
  • All manuscripts must be reviewed and approved by FARE at least 30 days before they are submitted for publication.
  • FARE will communicate outcomes from FARE Clinical Network data back to registered patient users.
  • FARE cannot be held accountable for the accuracy of information provided by users.

Distribution of de-identifiable specimens/data:

Distribution of specimens/data that are coded, but not directly identifiable, when the recipient researcher will not seek to identify the individual from whom the specimens were obtained, is not considered human subjects research. However the recipient researcher must agree to never attempt to access identifiable health/medical information or to attempt to identify the subject(s) who provided the specimen/data. Such coded specimens/data may be distributed without separate, independent IRB approval once the recipient researcher confirms the agreement stating that s/he will not attempt to identify human subjects from whom the specimens/data were derived.
